A number of SDSU College of Extended Studies' professional certificates have been approved for Veterans Education Benefits. Eligible veterans and family members eligible for education benefits will need to obtain authorization from the Department of Veteran Affairs prior to enrollment in a certificate program. CES Programs Approved for Veteran Education Benefits
First Time-Extended Studies Students Must Provide:
Chapter 30, 32, or 34
Montgomery GI Bill
- A copy of your DD214, (Not needed if still on active duty)
- If you have never used your VA benefits before, submit form 22-1990
- If you have previously used your VA benefits, submit form 22-1995
- CES Veteran Education Benefits Responsibility Sheet
- Include official transcripts from all schools you have previously attended following high school.
22-1990 - Application for VA Education Benefits - Required of all new applicants utilizing benefits for the first time. Eligible participants include veterans and reservists/guardsmen. Veterans should submit DD Form 214 (Member 4 Copy) and reservists/guardsmen should submit DD Form 2384 with application.
22-1995 - Request for Change of Program or Place of Training - Required of veterans and reservists/guardsmen having used benefits previously or who have changed degree/major since last enrollment.
CES Veterans Responsibility Sheet - Required of all veterans, reservists/guardsmen, family members, and eligible persons using veteran education benefits. This form informs you of your responsibility to maintain eligibility for Veteran Education Benefits.
Chapter 33
Post 9/11 GI Bill

Survivors' and Dependents' Educational Assistance Program (DEA)
22-5490 - Application for Survivors' and Dependents' Educational Assistance Required of dependents and spouses of totally disabled or deceased service-connected veterans.
22-5495 - Request for Change of Program or Place of Training Survivors' and Dependents' Educational Assistance Required of dependents and spouses of totally disabled or deceased service-connected veterans having used benefits previously or who have changed degree/major since last enrollment.

Montgomery GI Bill - Selected Reserve (MGIB-SR)
- If you have never used your VA benefits before, submit form 22-1990
- If you have previously used your VA benefits, submit form 22-1995
- NOBE - Notice of Benefit Eligibility DD Form 2384 (Your Reserve Unit must issue this form)

Active Duty Service Members Using Tuition Assistance
As an active duty service member you may be eligible to use your Tuition Assistance for College of Extended Studies Certificate Programs. For full details on how to use your Tuition Assistance, please visit your command or unit education officer.
Tuition Assistance Students Must Provide:
- Tuition Assistance Authorization Form (issued by command education officer)
- College of Extended Studies Registration Form
